Formsite is an online form building platform used to create and publish forms for collecting information from website visitors. Forms are created, configured, and managed within the Formsite dashboard.

When used with ShopWired, Formsite forms are embedded into website pages using JavaScript embed code, allowing visitors to complete and submit forms directly on your site.

1. Create a Formsite account
2. Build and configure your form in Formsite
3. Copy the JavaScript embed code provided for the form
4. Add the embed code to a ShopWired page using a Custom HTML section
5. Publish the page to display the form on your website
