Cognito Forms is an online form builder used to create forms for collecting information from website visitors.

Forms and form settings are created and managed within the Cognito Forms platform. Published forms are displayed on your ShopWired website by adding Cognito Forms embed code to website pages.

1. Create a Cognito Forms account if you don't already have one
2. Create and publish a form in Cognito Forms
3. Obtain the embed code for the published form
4. Add the embed code to a ShopWired website page
5. The form is displayed on your ShopWired website
